Before filing a written statement of complaint, complain directly to the person, firm, or owner of the company that sold you the item or performed the service. Describe the nature of the complaint and what action you would like them to take. Keep a record of your efforts to resolve the problem. Allow time for them to resolve the problem. If unsuccessful, use this form. Send it in with copies of any contracts, receipts, and other documents you may have to support your complaint. A copy of this form will be kept in our files. A copy of this form will also be sent to the business the complaint is directed against for their position and possible resolution. Consumer Affairs does not have jurisdiction over many areas of the law, and it may be necessary to transfer your complaint to the proper agency that can best address your complaint.
