Community Health Worker Program

Responsibilities

The Community Health Worker Program conducts education, referral and case management activities through targeted outreach and home visits in the Cities of Newburgh, Middletown and Port Jervis to provide information and services to individuals who might not otherwise contact a traditional health care provider.

Perinatal (the period around the time of birth) care and parenting concerns are the focus of this program. Outreach workers provide one-on-one and family education on disease risk reduction, safer sex practices, and encourage screening, and early detection of HIV, TB and other infectious diseases. Clients are referred to medical, dental and family planning services.

Case management includes assessing the health needs of the entire family as well as the client. Outreach workers assist in removing barriers to accessing healthcare services and helping to facilitate involvement with appropriate support groups.
